This is an adapter that connects to the factory shark-fin satellite antenna found in many Lexus and Toyota vehicles. You would use this if you are installing an aftermarket satellite kit but want to utilize your existing factory sharkfin antenna. It does not normally connect at the back of the radio. (With a few exceptions.) It typically connects at the antenna location. This may involve removal of your headliner.
It is 12 feet long and has a female connector on one end and a standard SiriusXM tuner connection on the other. The female end is designed to plug into the male connection found in the vehicle. The plug on this cable is pictured in this ad. If the plug in your vehicle looks exactly like this one, you need the male version (SFA12M) instead.
